Complex Care & Family Support

Our Complex Care Consulting service provides expert guidance for patients and families facing complex medical or cognitive challenges. Integrated within our therapy and wellness practice, this service offers education, resource navigation, and strategic planning to help individuals safely remain at home or transition to appropriate housing.

Families facing medical or cognitive challenges often struggle to find the right information and resources to make informed care decisions. Our Complex Care Consultation service provides expert guidance to help families understand available support options, navigate housing and care resources, and improve their knowledge of disease progression—all while maintaining control over their own decision-making process.

Key Areas of Support:

✔ Housing & Care Options – Guidance on modifying the home for safety or exploring alternative living arrangements.

✔ Resource Navigation – Connecting families with home-based support services, respite care, and community programs.

✔ Disease Education – Helping families understand conditions like dementia, including expected progression and care needs.

✔ Support for Caregiver Communication and planning – Providing tools to align caregiving decisions and set realistic expectations.

This service complements our therapy and wellness services by addressing broader care concerns, ensuring patients and families feel confident about their next steps.

Please note, we do not oversee care, advocate on behalf of clients, or monitor ongoing needs. Instead, we equip families with the knowledge and resources needed to make informed decisions independently. All meetings are held virtually and can accommodateup to four people at a time. Complex Care + Family Support Services are provided under our Wellness365 Program and are considered a Non-Covered Service by Insurance; all payments must be made at time of service.

Jennifer Pilz, MSPT, CMC, is a licensed physical therapist and certified care manager with over 20 years of experience in the aging and disabilities sector. She brings a unique blend of clinical expertise and care consulting to Healthy Aging PT, helping individuals and families navigate complex care needs with confidence.

As the founder of Riverside Care Advisors, LLC, Jennifer has dedicated her career to enhancing the well-being, independence, and dignity of older adults through education, resource navigation, and expert guidance. Her background in geriatric care management, healthcare consulting, and physical therapy allows her to assess and address a wide range of needs, empowering families to make informed care decisions. Jennifer is also a passionate public speaker and advocate, committed to connecting individuals with high-quality resources to support aging in place, housing transitions, and chronic condition management. At Healthy Aging PT, she continues her mission of helping individuals achieve optimal health, function, and quality of life through expert consultation and strategic planning.
