Hospitalization + Extended Absence Policy
EMERGENCY ROOM VISITS AND PLANNED AND UNPLANNED HOSPITALIZATIONS LASTING LESS THAN ONE WEEK:
If you require emergency care, or are hospitalized during your HAPT episode of care, please let us know as soon as possible by calling the main number (617) 398 - 4508. If you able to return home within one week, and do not require care from a Medicare A Home Health Agency, we would be happy to resume your therapy at that time. Please note: If we do not hear from you within one week from your last scheduled visit with us, we will automatically discharge you from our care.
PLANNED AND UNPLANNED HOSPITALIZATIONS LASTING MORE THAN ONE WEEK:
If you experience a planned or unplanned hospitalization lasting more than one week, please notify us as soon as possible by calling (617) 398-4508. We will need to discharge you from your current HAPT plan of care. This ensures there is no overlap of services with any Medicare A Home Health services, which you will likely require upon discharge. It also allows us to keep our therapists' schedules consistent and offer open appointments to patients currently on our waitlist.
If you discharge from the hospital to an acute or skilled rehab facility, and return home under the care of a VNA (Medicare A Home Health), we ask that you or your VNA care team contact us directly once a discharge plan is in place. While we are always happy to resume your outpatient-at-home care, we cannot hold a spot on our schedule until we are provided with an estimated discharge date. Please remind your VNA team that we often book out 1–2 months in advance. The sooner they are able to communicate your expected discharge date, the more likely we will be able to resume your care without delay. Once we receive an estimated discharge date, we will do our best to schedule your next visit as promptly as possible. We suggest sharing the following information with your VNA Team:
“I was working with a PT or OT from Healthy Aging Physical Therapy and would like to resume my outpatient-at-home care when I discharge from the VNA. Please call (617) 398-4508 or email Katie@HealthyAgingPT.com when you know my likely Agency Discharge Date, so they can schedule my care accordingly.”
OTHER EXTENDED ABSENCES: If you require an extended hold from therapy, whether due to illness or travel, we will determine whether we can put your care on hold, or if we feel it would be better to discharge and resume therapy upon your return. Your therapist and team will determine this on a case-to-case basis.
