Healthy Aging Physical Therapy is committed to protecting the privacy and security of your health information while making it easy for you to access your medical records when you need them. Please complete the form below to request a copy of medical records maintained by Healthy Aging Physical Therapy.

Who May Request Records?

A request may be submitted by:

• The patient

• A parent or legal guardian, when legally authorized to act on behalf of the patient

• A health care agent, health care proxy, or other legally authorized personal representative, when their authority is currently in effect and applies to the records requested

• Another individual or organization specifically authorized by the patient to receive the records

Note: If you are requesting records for another person and are not their legally authorized representative, the patient must complete and sign the request authorizing Healthy Aging Physical Therapy to release the records to you.
